This form is for applying for a new address. If you need to order replacement signs for existing addresses please complete a Rural Address Sign Replacement Order.

In order to complete this form you will need the following information. Incomplete/incorrect forms will delay the processing of the address application.

  1. Your contact information
  2. Whether or not the property has a joint (or shared) driveway
  3. Location of the property including the town, road name, and a parcel number or tax ID
  4. Measurement from the nearest intersection to the driveway location. This should be from the centerline of the road to the center of the driveway.
  5. A sketch, map or site plan is optional, but greatly appreciated as it can help reduce the chance of error and misinterpretation.
  6. Confirmation from Town official. (This can be a: zoning permit, building permit or letter/email from a Town Official.)
