Community partnerships refers to our relationships with organizations or groups that align with our mission of improving population health and removing the barriers that prevent people from reaching their full health potential. We currently have partnerships with a variety of local organizations, including nonprofits, businesses, libraries, schools, healthcare systems, faith-based organizations, and local municipalities.

We work closely with other Rock County departments, including Rock County Human Services, Emergency Management, the Sheriff’s Office, and various County committees. We also work with the Wisconsin Department of Health Services, and other public health departments throughout Wisconsin, to further our work in Rock County.

Our Work in Action

We are proud to facilitate the work of the Health Equity Alliance of Rock County (HEAR), and the Nutrition Collaborative of Rock County. Each of these groups include members from local organizations, businesses, non-profits, civic groups, volunteers, and Rock County residents. Click on each tile at the bottom of this page to learn more about both groups.

Some of the ways we work with our community partners include:

  • Providing guidance and support on communicable disease prevention
  • Organizing events or initiatives such as the prescription drug collection program
  • Sharing information and resources on breastfeeding
  • Collaborating on grants focused on diabetes and heart disease prevention
  • Working on large-scale projects such as the Community Health Improvement Plan

We also support and participate in other local groups to help improve mental health, reduce the impacts of substance use, promote diversity, equity, and inclusion, and much more. Impact on Rock County

Strong, supportive partnerships with local and state partners are key in reaching our mission of improving Rock County's population health. We partner with organizations throughout Rock County and Wisconsin to help ensure we're able to make the biggest impacts with our programs and projects.
